Please be prepared to pay for procedures on the day of the service. In an effort to keep our operating costs down and pass those savings along to our patients, we offer a full payment courtesy for our larger cases paid in full by cash or check on the day we begin your case. If you have dental insurance, please bring your card with you so that we may make a copy for our records. We accept cash, check, Visa, MasterCard, Discover, American Express, and Care Credit. |
| While our office will strive to ensure use of your maximum benefit available, you are legally responsible for payment in full for all services rendered. As a courtesy, our office will complete and submit your insurance forms to achieve the maximum reimbursement to which you are entitled. Your insurance is a contract between your employer and a dental insurance company. Benefits received are based on the terms of the contract negotiated between these two parties. We urge you to read your insurance policy so that you are fully aware of coverage and any limitations of the benefits provided. |
